Whether you are a patient making your own referral or a physician making a patient referral, you can start that registration process below.
New Patients – Making Your First Appointment
You don't need to have a physician's referral to receive a cancer screening or to be treated at Moffitt. Complete our online Self-Referral form or call our New Patient Appointment Center at 813-745-3980 (Toll Free at 1-888-860-2778) anytime between 7 a.m. and 7 p.m Monday-Friday and most holidays. Your online request will be responded to within one hour during business hours (8 a.m. to 5 p.m.).

Refer a Patient- Referring Physicians
Physicians can also refer patients to us by filling out our onlinePhysician Referral form or by calling 813-745-3980 (Toll Free at 1-888-860-2778) option #1 anytime between 7 a.m. and 7 p.m Monday-Friday and most holidays. Your online request will be responded to within one hour during business hours (8 a.m. to 5 p.m.).
Moffitt also offers a Physician Resource Line for questions and consultations at 1-888-MOFFITT (1-888-663-3488), option #1, available between 8 a.m. and 5 p.m. Through the physician resource hotline, referring physicians can speak directly to a Moffitt physician. Calls will be answered by Registered Nurses. Requests to speak to a Moffitt physician about a patient referral will be assisted immediately and completed within 24 hours.
